2.  Recent 2-inch x 2-inch photo with white background 2 Copy
3.  NID Card and Utility Bill Copy
4.  Original Bank statement (last six months) / $200 USD Endorsement
5.  N.O.C./G.O/ Trade License Notarized Copy with Translate in English & Letterhead Pad
6.  Student ID Card and Birth Certificate (if child travels with parent)
